I use companies like CallCentric, Les.net, InPhonex, and Voip.ms.
These are all BYOD (Bring Your Own Device) voip services, which means you have to have enough tech savvy to provide and install and configure your own voip equipment. These companies are often used by business that install and maintain their own IP-PBX systems, but home users like myself can also use their services if you can install your own ATA (analog telephone adapter) and configure it yourself.
I configure my own Linksys SPA2102-NA adpater and have been using it for a couple years now with all of the services listed above, but have my DID phone numbers provided through CallCentric with very good results.
CallCentric also provides calling card gateways which allows you to make voip calls through their network by calling local gateway numbers located in major cities around the world. This way you get voip rates even when calling from a regular PSTN landline, or cell.

Highest quality from what I've heard is Vonage. There are less expensive options but you may sacrifice features. I had Comcast digital voice for ~30 a month, now I have Tmobile for ~10/mo. But now I have no web-voicemail and it can't be used for faxing. There is a thing called magicjack which claims to give you home VOIP service for a year for ~$30 but there's no human support, which is obvious given it's low price. So, I think the leader in the industry is Vonage, but they're not the cheapest.

Free PC-2-Phone calls to over 45 countries - but you still gotta recharge, at first, for 10euros - cuz not every country is free. Also, to call the free countries, you'll need to recharge and keep some money in yer account.

Here: http://www.internetcalls.com

Download the messenger, recharge with yer credit card, and the voice quality matches SKYPE - I love it! And you get yer own free phone-in no. which SKYPE charges like 30 dollars or so for - Yahoo! charges some 3 dollars - here it is free.

You'll be surprised - just like I was - itz awesome.

VoIP service should work just like plugging in a phone to the phone jack. If you plug in a phone to the router and pick it up you should hear a dialtone. If you do not hear a dialtone, the voip service may not be properly enabled by your internet provider. There is no special phone that works, I have tested it on over 20 types of handsets including panasonic, sony, uniden and all the other major brands. It works on both corded and cordless phones (for cordless the base unit must be plugged in to the router).

I would tell you to call customer service, but that may be a bit of a problem if you do not have phone service yet. If you have a cell or another way to make a call, I would do so and you will need to be at your computer and have access to the router as well when you call.
